You may become an organ donor no matter what age you may be. If you are younger than 18, you will need to get your parent's permission. Here's what you'll need to do in order to make sure your organs will be donated in the event of your death:

Download, print, complete and mail the donor registration form found here: http://www.mwtn.org/html/admin/donorregistry.pdf

Fax or mail completed form to: Missouri Organ Donor Program

Missouri Department of Health and Senior Services

PO Box 570

Jefferson City, MO 65102-0570

Phone: (toll-free) 888-497-4564

or Fax: 573-522-2899

A letter of confirmation will be mailed to you within 30 days of receipt in the Missouri Organ Donor Program.

Next time you renew your driver's license or state ID, be sure to tell the DMV that you have become a donor.
