If you are 18 or older (or 16-17 and have your parents' permission), you can take part in the organ donor program by letting the staff member at the driver licensing office know your wishes when you apply for your driver license, instruction permit, or identification card. The “heart” will appear on your license or identification card and your name will be transmitted to the organ donor registry.

You can also go to the organ donor website at http://www.livinglegacyregistry.org/home.html and add your name to the registry and click on "Become a Donor" to register online.
